This page contains specific information on how to use this exciting and innovative care. Please read it carefully and call us if you have any questions.
Your enhanced access and personalized services begin as soon as your membership starts, which is January 1, or whenever we receive your payment for those paying after January 1. For those paying monthly by credit card, this payment will occur on the first of the month. For any payment, billing, or insurance questions, please call Cathy Knoth at 425/424-2201 or e-mail her at cathyk@CanyonCreekClinic.com. We are here to help you.
Member Benefits:
Your membership includes 24 hours per day seven days per week direct access. There are multiple ways to reach us, depending on the urgency of your need and the best method of communication for you.
Current regularly scheduled clinic hours are Monday 9 AM-5 PM, Tuesday 1 PM-8 PM, Wednesday 9 AM-1 PM, Thursday 9 AM-5 PM, Friday 7:30 AM-4 PM, Saturday 10 AM-1 PM. You will be seen at other times as needed.
How to Reach Dr. Blume:
You can reach Dr. Blume by e-mail, reserved for direct practice members, at gblume@CanyonCreekClinic.com. You should expect a reply to your e-mail within 24 hours, except in the unlikely event of technical e-mail problems, or when Dr. Blume is on vacation, the dates of which will be posted in advance here on our website in the direct practice members exclusive section. To access the direct practice members exclusive section, your user name is Canyon, and your password is Creek. Remember that e-mail is not totally secure. Also remember that we cannot reply to you by e-mail if you block our e-mail using a spam blocking system. By sending us e-mail, you agree to allow us to reply via e-mail unless you state otherwise in your message. Do not use e-mail for urgent issues. Only use e-mail for issues appropriate for a 24 hour turnaround.

Please understand that she may not be able to answer immediately when you call or text if she is taking care of another patient.
You can reach Dr. Blume directly via cell phone outside of regularly scheduled clinic hours by calling or texting 206/473-7669. Please understand that he may be with a patient but will respond as soon as he completes their visit.

Please check out our website for practice updates and other information. We will be sending you regular newsletters and will be creating regular updates regarding medical topics of general interest, which are designed to give you information you can use.
Preparing for Your First Visit:
A very important feature of your direct practice membership is the in depth, personalized, preventive care package. The exact details of this care will vary depending on your specific situation, but this includes a detailed evaluation of your personal health risks and opportunities, with a personal action plan and follow-up. The initial visit will be approximately 2-2 1/2 hours in length. Please schedule this visit as detailed above, but please indicate that the requested visit is for your initial wellness visit so that we can schedule the correct amount of time.
It is crucial that you begin to fill out the online initial wellness visit form at least two weeks prior to this visit. Please tell us at the time of your scheduling request if you want these forms to be mailed to you instead of accessing them online, and please mail them back at least one week prior to your visit. These forms ask for extensive personal information and will take significant time to complete. Please take the time to fill these forms out thoughtfully and accurately. If there is any information you can’t answer, or are uncomfortable sharing, please leave the area blank. If you are uncertain of some exact information, don’t worry, just fill in the information with what you currently know. It will significantly hamper our ability to provide you with superior preventive care if you do not complete this information.
Please do not eat anything for 8 hours before your visit. Please drink a reasonable amount of zero calorie fluids before the visit, including water, any zero calorie diet drink, black tea or black coffee. Diabetics, please bring something to eat with you, although if this is not practical we will have something for you. If possible, wear a sports top and shorts under your normal visit clothing.
On this first visit we will review your family and personal health histories, diet, exercise, and sleep log, goals and concerns, perform an extended physical exam, measure body composition analysis, perform total body photography, do carotid IMT testing, and draw blood work customized to your situation. We will additionally treat any acute issues. At the conclusion of this visit, a follow-up visit will be arranged in approximately 3 weeks.
Preparing for Your Second Visit:
At your follow up visit, we will discuss the findings of the initial visit, you will receive a CD with your total body photography pictures for skin self examination, you will receive your Vital Key personal portable medical record, you will have magnification views (dermoscopy) with computer analysis performed on any suspicious pigmented skin lesions identified by the initial total body photography, and we will jointly finalize a specific wellness plan with follow up. If indicated, you will receive an activity monitor with instructions in its use. We will mail or email the finalized version of your plan to you.
We will contact you would at appropriate intervals for follow-up visits to monitor your progress to ensure successful completion of your individualized wellness plan. If you have any questions, please contact any of our staff, Angie, or Dr. Blume as indicated above.
